Attention MGH Youth Scholar Alumni: The purpose of this form is to provide uAspire with your information for the 2025-2026 MGH Youth Scholars Scholarship. Scholarship amounts can change year to year depending on changes in your Student Aid Index (SAI), college costs, and other variables. uAspire will use the information you submit on this form to provide an assessment of your financial need for the 2025-2026 academic year. 

Please keep in mind:

Students are expected to pay their SAI. The SAI is calculated when you complete the FAFSA and can be found on your FAFSA Submission Summary. 

Federal Stafford loans are included in scholarship calculations. Students are not required to take out student loans, but if you choose not to, you are required to cover the gap out-of-pocket.

In the case that your financial need is determined less than $1,000, you will receive $500 book checks sent directly to you each semester of this academic year.

To complete this form you will be required to submit:

       1. 2025-2026 financial aid offer (award letter) and FAFSA Submission Summary

       2. If you were enrolled in college, your most recent cumulative and semester GPA averages
